form, the header, for your websites. One can Google---State of California Claim
form for losses, and then click on pdf information and claim form.
Claims
can be for losses one believes were caused by the action, or inaction, of
any state agency.

can be for temporary relocation expenses due to the CDFA LBAM aerial/ground spray
program (i.e. travel, hotel, meals), loss of wages due to relocation
because of the CDFA LBAM aerial/ground spray program, costs to clean property
(homes, yards, play equipment, autos, etc.) due to contamination from the CDFA
LBAM aerial/ground spray program, medical costs including doctor's and hospital
visits, veterinary costs for pets and farm animals and any other reasonable costs
that can be attributed to the CDFA LBAM aerial/ground spray program.
Please
review the 24 instructions for filling out the form in the printout before submitting
claim form to the State. A $25 filing fee is required. You may wish to consult
an attorney for assistance with filing a claim.
